The goods and services shown within are those we can provide to our customers.  You may choose only the items you desire.  However, any funeral arrangements you select will include a charge for our basic services and overhead.  If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement of funeral goods and services we provide describing the specific services you have selected.

BASIC SERVICES OF FUNERAL DIRECTOR AND STAFF
Our fee for the services of funeral director and staff includes, but is not limited to, staff to respond to initial request for service, arrangement conference with family or responsible party, preparation and filing of necessary authorizations and permits, recording vital statistics, preparation and placement of obituary notices, planning of funeral arrangements and coordination of service with those providing other portions of the funeral,  e.g. cemetery, crematory and others as required. Also included in this charge are overhead expenses relative to our facility such as insurance,  maintenance and utility expenses, secretarial and administrative costs, and equipment and inventory expenses, e.g. vault haulage and installation, tent if needed, casket selection rooms and other inventory items.
This fee for our basic services and overhead will be added to the total cost of the funeral arrangements you select. (This fee is already included in our charges for direct cremations, immediate burials, and forwarding or receiving remains.)

Use of Facilities for Funeral Lunch
If time for refreshments is wanted at the funeral home following a funeral or memorial service we will provide the facilities and tables and chairs needed as well as clean up and garbage disposal. *food is not provided

EMBALMING
Except in certain special cases, embalming is not required by law.  Embalming may be necessary, however, if you select certain funeral arrangements, such as a funeral with viewing.  If you do not want embalming, you usually have the right to choose an arrangement that does not require you to pay for it, such as direct cremation or immediate burial.

FORWARDING REMAINS TO ANOTHER FUNERAL HOME
This charge includes our basic professional services of staff, removal and shelter of remains, embalming, filing the death certificate and issuing permits, transportation to airport or railroad depot, and receptacle for shipping.  This fee does not include any services of the staff for funeral services or viewing prior to forwarding the body.

This charge includes our basic professional services of staff, shelter of remains, transportation from airport or railroad depot, transportation to local cemetery or crematory.  This fee does not include any services of the staff for funeral services or viewing.

IMMEDIATE BURIALS
Our charge for an immediate burial, without any services, ceremonies or reviewal, includes removal and shelter of remains, local transportation to the cemetery, necessary authorizations and basic services of staff.

These prices do not include the cost of embalming, a casket, an outer burial container which may be required by the desired cemetery, merchandise, or any cash advance items.

DIRECT CREMATION
Our charge for a direct cremation without any services, ceremonies or reviewal includes: basic services of funeral director and staff; transfer of deceased; cremation casket/alternative container; basic urn; remembrance jewelry (select packages) and Living Memorial Program with select packages.  Alternative containers encase the body and can be made of materials like fiberboard or composition materials (with or without an outside covering.) The containers we provide are unfinished cardboard containers.

DISCLAIMER OF WARRANTIES
Bakken-Young  Inc. does not make any representations or warranties regarding caskets, outer burial containers, or other funeral goods sold by the funeral home.  The only warranties expressed or implied, granted in connection with the goods sold with this funeral service are the express written warranties, if any, extended by the manufacturer thereof.  No other warranties, express or implied, including the implied warranties of merchantability or fitness for a particular purpose, are extended by Bakken-Young Inc.

PAYMENT TERMS
Payment of the total amount is due in full within 30 days when cash advance amount is paid at the time of arrangements.  If the total amount is not paid by the due date, a penalty of 1.66% per month (20% per annum) for LATE PAYMENT  will be charged on the unpaid balance.  Purchaser agrees to pay all costs and reasonable attorney’s fees incurred in collecting the amount due if not paid in full by the due date.  A 3% discount on services and merchandise ONLY, will be given for all accounts paid by the day after the service provided.

PRE-ARRANGED AND PRE-FUNDED FUNERALS
Many people are discovering that pre-arranging their funeral services lightens the burden on family members when the need arises.  Pre-arrangement and pre-financing of funerals is a  rising trend nationwide.  There are two options available for pre-arrangements as dictated by Wisconsin state law.

Insurance Trust Accounts
Also 100% trusted, with the depository being an insurance company.  All interest accumulates to the account until drawn out with a certified copy of the death certificate.  There are several insurance companies who handle funeral trusts.  They are originated with the funeral home and insurance trusts do not have the same limits as bank trusts. No 1099 issued.

Bank Trust Accounts
All monies received by the funeral home are placed in a 100% trust through a local bank in a certificate of deposit.  All interest accumulates to the account until drawn out with a certified copy of the death certificate.  There are limits to the amount that can be deposited in a bank trust.  And a 1099 will be issued annually.

Pre-arrangement and Medical Assistance
It is often necessary to place a person on Medical Assistance as their assets dwindle.  When funds are running out, a spend-down of remaining assets begins.  An acceptable spend-down is a funeral pre-paid agreement.  There are certain limits which must be adhered to.  Our funeral directors can advise you on the ways to place funds in trust and qualify for Medical Assistance programs.
